1、進口非特殊用途化妝品備案申請表;

2、產(chǎn)品配方；

3、產(chǎn)品質量標準;

4、經(jīng)衛(wèi)生部認定的檢驗機構出具的檢驗報告及相關資料，按下列順序排列：A. 檢驗申請;B. 檢驗受理通知書;C. 產(chǎn)品說明書;D. 衛(wèi)生學(微生物、理化)檢驗報告;D. 毒理學安全性檢驗報告；

5、產(chǎn)品原包裝(含產(chǎn)品標簽)。擬專為中國市場設計包裝上市的，需同時提供產(chǎn)品設計包裝(含產(chǎn)品標簽);

6、產(chǎn)品在生產(chǎn)國(地區(qū))或原產(chǎn)國(地區(qū))允許生產(chǎn)銷售的Z明文件;

7、進口化妝品報關需提供其中文標簽、貨物入境檢驗檢疫通關單、產(chǎn)品成分表、contract、invoice、裝箱單和報檢、報關委托書等手續(xù)。

4. The inspection report and relevant data issued by the inspection institution recognized by the Ministry of health are listed in the following order: A. inspection application; B. inspection acceptance notice; C. product specification; D. hygiene (microorganism, physical and chemical) inspection report; D. toxicology safety inspection report;
5. Original packaging (including product label). Product design packaging (including product label) shall be provided at the same time if it is planned to be specially designed for the Chinese market;
6. Z-document that the product is allowed to be produced and sold in the country (region) of production or the country (region) of origin;
7. For customs declaration of imported cosmetics, the following procedures shall be provided: Chinese label, goods entry inspection and quarantine customs bance b, product comb list, contract, invoice, packing list, inspection declaration, customs declaration power of attorney, etc.
